Voluntary Short-Term Disability Insurance (Employee Paid) replaces a portion of your income when you are unable to work due to illness or injury. This coverage provides a weekly benefit that equals 60% of your weekly earnings up to a $1,500 weekly maximum. Benefits become available after you have been disabled for seven (14) days due to illness and injury (benefits begin on the 15th day). Benefits are payable for a maximum duration of 11 weeks. This benefit has a 6 month pre-existing condition look back period for benefits.

If you remain disabled for 90 consecutive days due to any one disabling illness or injury, you will be eligible to receive Long-Term Disability (Employer Paid) benefits. If approved by the insurance carrier, this coverage will provide a continued source of income that equals 60% of your monthly earnings to a maximum of $8,000 per month. Long-Term Disability will continue to be payable up to the Social Security Normal Retirement Age. This policy does have a 12 month pre-existing condition look back period for benefits.

Both Short-Term and Long-Term Disability are provided to employees through Mutual of Omaha.
